Our fax number is 770-552-1228.  Please include the following information on your faxed copy. 

1. Include your name as it appears on the card

2. Identify the type of card.

3. Give us your card number.

4. Give us your expiry date.

5. On the back of the card is a 3 digit security code that must also be provided.
